TARGETED CASE MANAGEMENT
Targeted Case Management of Kansas (TCM) assists individuals, their caregivers, and support networks with identifying, developing, selecting, obtaining, coordinating, utilizing, and monitoring paid services and natural supports. The goal is to ensure each person has what they need to thrive through personalized planning and ongoing guidance. Individuals have a choice in the providers they choose, and the CDDO can provide a comprehensive list of all affiliated providers for them.

PERSONAL CARE ATTENDANTS
The Personal Care Attendants, LLC agency enables adults with intellectual and developmental disabilities, such as Down syndrome and autism, to live and work safely on their own with individualized support. We work with clients individually to support them in accomplishing necessary activities of daily living, such as meal planning and preparation, grocery shopping, apartment care, laundry, and organizational tasks, as indicated on individual skill plans. Skill plans are implemented using strategies such as oversight, cueing, reminder systems, assistance, and modeling.
